Grade Change Form


Security to the grade change form is limited to one or two users per academic unit or department.  Access is limited in order to prevent fraudulent grade changes from being made.  If you are not able to access the form, please contact the administrative assistant in your department or unit for a paper copy of the form.

All grade changes must be submitted on the paper form with original signatures from the instructor, department chair and dean (or representative).  Please contact the Assistant Registrar at 816-235-1216 if you need additional information.
